WALK IN SERVICE: A Walk-In Service is currently in effect for Workman's Permit transactions. Senior Citizens wanting to conduct renewals or any driver's licence transactions can also make use of the Walk-In Service. Appointments are no longer required for these transactions.
For the application of a Workman’s Permit, the applicant shall submit the following documents:
a. Letter of application on Company’s Letterhead requesting approval for permission to own a Workman’s Permit
b. A Completed application Form No. 4
c. Certified Copy
d. Valid Certificate of Inspection
e. Company Registration
f. Evidence of Workers
g. Insurance covering additional passengers
There is a Monthly Fee of $90.00 per month (Usually issued in period of six (6) months)
