Class 4 & 5 Applications (Heavy T)

The applicant must be at least twenty-one (21) years of age and the holder of a driving permit of the next lower class of motor vehicle. For example, a class 4 applicant must be a holder of class 3, and a class 5 applicant must be a holder of class 4, for no less than one (1) year prior to the application. The following documents are required to be submitted: 

1. A completed applicant Form No. 4 available at Licensing Authority Transport Division’s offices;
2. A valid Driver’s Permit;
3. A completed Form No. 12 “Medical Test for Driver’s Permit” for which an applicant must produce a recent passport size photograph prior to the application, if applicable; and
4. Four (4) passport size photographs with green background (full frontal view of the face ears in viewing)
